Job Description

We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and Downtown East area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, harm reduction and homeless programs, addiction and recovery services, Pathways to Education, infant and child development programs, parenting programs, and youth employment services. We currently have partnered with the Downtown East Toronto Ontario Health Team, (DET-OHT) to develop and implement an Interprofessional Primary Care Team. The Interprofessional Primary Care Team (IPCT) is an initiative by the DET-OHT that brings together Regent Park CHC, St. Michaels FHT, Inner City FHT, and Sherbourne Health FHT to enhance access to interdisciplinary primary care for individuals in the downtown east of Toronto, including Moss Park, St. Jamestown, and Regent Park, who face significant financial challenges, housing insecurity, and other forms of marginalization. The DET-OHT comprises 43 health and social service organizations deeply rooted in the community, aiming to provide seamless, inclusive, and high-quality care through an integrated system. The DET-OHTs initiatives focus on priority populations, such as people experiencing homelessness and those with mental health and addiction issues, guided by principles of client and family-centered care, health equity, diversity, transparent collaboration, quality improvement, and responsive leadership. Job Summary The Nurse Practitioner will support client attachment to primary care services. The NP will work closely with the DET-OHT IPCT central navigation team and the four primary care organizations within the DET-OHT to deliver integrated services, with an emphasis on health stabilization and fostering strong, ongoing attachment to primary care. The Nurse Practitioner will provide comprehensive primary care to a diverse population facing significant barriers to healthcare, assisting patients in managing chronic conditions, preventing illnesses, promoting health, and effectively navigating the healthcare system. Three Year Contract Full Time 35 hours per week Salary Range: $112,012 to $134,936, plus opportunity to participate in HOOPP (Healthcare of Ontario Pension Plan).
