The pharmacist will work at the top of their license under a broad collaborative practice agreement to play a crucial role in providing comprehensive medication management services to our complex autoimmune patient populations. This specialized pharmacist will focus on optimizing medication therapy to improve patient outcomes, reduce hospital readmissions, ensure patient access to medications, and enhance the quality of life for our patients through autonomous face-to-face and telehealth visits with patients. This pharmacist will collaborate closely with other healthcare professionals to ensure comprehensive and collaborative care as well as work on process improvement opportunities. They also will be integrated into our well-established ambulatory pharmacy team so they can collaborate with other ambulatory clinical pharmacist specialists in other markets and be part of the pharmacy team at SSM Health. You will be expected to be on site in clinic the majority of the days you work but can accommodate some flexibility with remote coverage when necessary.If you have a passion for providing high-quality, direct patient care, want to practice at the top of your license in an ambulatory setting, and have advanced training or experience in autoimmune disease states, this is the position for you!Job Summary:Assumes an expanded pharmacist role by providing direct patient care and may practice with approved privileges granted through collaborative practice agreements with Medical Staff. The role will be actively engaged in all complexities of patient medication management, monitoring, and education, as needed. Participates in the training and education of students, residents, and physicians as applicable.Job Responsibilities and Requirements:Primary ResponsibilitiesGathers necessary data to promote patient safety by ensuring that each relevant medication is indicated, effective, safe, and that the patient is adherent to therapyImproves patient-specific drug therapy and therapeutic problems by evaluating the appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects.Assesses relevant patient-specific medical problems, prioritizes medical problems based on urgency and severity, and identifies preventive and health maintenance issues as needed. May design a drug therapy plan for patient-specific problems and independently implements the plan.Provides enhanced patient care through detailed counseling on medications and disease states, adherence assessment, medication cost analysis, review of immunization status, and communication with patients providers and pharmacy.Provides expert consultation with other health care professionals in the development of drug-related treatment plans, assessment of drug therapy, outcomes and identification and resolution of drug-related problems.Identifies patient outcomes, correlates those outcomes to clinical processes, identifies processes that yield best outcomes, and assists in the implementation of those best practices.Participates in quality management programs to facilitate continuous process improvement initiatives and quality improvement projects.Precept pharmacy students and residents, as applicable.Works in a constant state of alertness and safe manner.Performs other duties as assigned.EDUCATIONPharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacyExperienceTwo years experience or completion of PGY1 residencyPhysical RequirementsFrequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.Frequent keyboard use/data entry.Occasional bending, stooping, kneeling, squatting, twisting and gripping.Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.Rare climbing.Required Professional License And/Or CertificationsResuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment.
