In this role you will support the development and lead the implementation of multiple Integrated Care Programs that uniquely focus on co-design with system partners.We offer the most competitive wages, paid sick days, RRSP match, employee wellness program and the opportunity to make a real impact.Job DescriptionAs the Program Director of Integrated Care, you will play a crucial role in developing and maintaining key relationships with our clinical and operational health system partners. Your primary objective will be to contribute to the implementation and sustainability of integrated care programs that drive positive health system change.Lead operational planning and implementation of multiple concurrent integrated care programsDefine service delivery requirements at the local level, collaborating with various external partners and patient populationsPromote and implement the 'One Team' approach, fostering collaboration and synergy across the organizationEnsure smooth program operations and continuously improve processesServe as the primary representative for integrated care programs, presenting to stakeholders and promoting program initiativesAnalyze program metrics and utilize findings to drive positive changeMentor and support staff to maximize their potential and enhance team performanceStay informed of industry trends and best practices to maintain program excellenceQualificationsA degree in a relevant health-related fieldActive designation as a Regulated Health Professional and current membership in good standing with the relevant professional college or associationA minimum of 5-7 years of leadership and management experience in complex healthcare organizationsDemonstrated expertise in healthcare operations, change management, and program development, particularly in environments involving multiple stakeholders and complex organizational structures.Comprehensive knowledge of Ontario's health system design and funding modelsA track record of successful implementation of integrated care programs and initiativesAdditional InformationSpectrum Health Care is thrilled to be named one of Canadas Best Managed Companies.
