As a member of the CCC, QHC Interprofessional team you will have the opportunity to provide care to the medically complex continuing care patient population.Job DescriptionAssessment of patients with communicative, swallowing and cognitive-communication dysfunction of the cardio-pulmonary, musculo-skeletal and neurological systems and co-creation of goals with the patient and familyPlanning and implementation of treatment plans to improve communication, swallowing ability and overall participation in life activitiesParticipate in patient discharge planningDocument patient assessment and treatment in the health recordCollaboration with the inter-professional healthcare team, patients and their families to achieve patient goals and augment the treatment plan as neededProviding education to the inter-professional team to support evidence based careAssigning to and supervision of support personnelInvolvement in clinical education of various professions, including preceptorship of SLP studentsQualificationsMasters degree in Speech-Language Pathology from an accredited institution.Licensed with the College of Audiologists & Speech-Language Pathologists of Ontario (CASLPO)Previous experience with assessment and management of communication and swallowing disorders in a hospital setting, including video- fluoroscopic assessmentMBSImp certification is preferred or willingness to be certifiedDemonstrated commitment to ongoing professional development and student learningStrong case management skills and demonstrated ability to work in a fast paced team environmentSuperior interpersonal communication skills, including verbal and written communicationInterest in potential professional and career development opportunitiesSatisfactory attendance and performance requiredExperience with acute, rehab and chronic adult communication and swallowing disorders in a hospital settingExperience with complex geriatric patients with co-morbid neurological, surgical and medical conditionsExperience with end of life care is preferredExperience with tracheostomies is preferredTo pursue this career opportunity, please visit our website: Candidates who believe they possess the necessary qualifications and experience for this position and who have been in their current position for at least six (6) months are encouraged to apply.Candidates are selected on the basis of their skill, ability, experience and qualifications.
