This 0.5 FTE position offers comprehensive assessments and care planning for individuals with complex chronic conditions such as: Chronic fatigue syndrome (CFS) Environmental illnesses including Multiple Chemical Sensitivity Functional neurologic and gastrointestinal syndromes Multiple chronic conditions The ICCS model focuses on the whole person, emphasizing self-management support. The team includes physicians, occupational therapists, a nurse practitioner, registered nurse, licensed practical nurse, clinical therapists, and a dietitian. This role is salaried, approximately $10,340 per month, with a flexible schedule. Physicians are expected to have regular weekly hours, providing 70% clinical care and 30% research activities, which are integral to the role. The position reports to the Head, Department of Family Practice, Central Zone, and the Director, Primary Health Care and Chronic Disease Management, Central Zone, along with the Program Manager.
