Occupational Therapist - Independent Contractor Home ProgramToronto, ON | Part-time (10 visits/week) | Community-Based (Home Visits Required)We're looking for a part-time Occupational Therapist to support patients transitioning from hospital to home in a community-based setting. In this independent contractor role, you'll provide in-home assessments, recommend assistive devices and adaptations, and help clients improve function, independence, and quality of life. As part of an integrated care team, youll focus on population health and chronic disease management, empowering patients to regain control of their daily activities post-discharge.
