Job Title: Occupational Therapist - Hospital to Home Program Location: Toronto, ON Setting: Community-Based / In-Home Care Employment Type: Full-Time, Permanent Start Date: As soon as possibleAbout the RoleWe are seeking a passionate and experienced Occupational Therapist (OT) to join our interdisciplinary care team on a full-time, permanent basis. In this role, you will work directly with clients in their homes across Toronto, providing assessments, treatment planning, and interventions that help individuals regain independence, mobility, and overall quality of life.You will support individuals recovering from hospital stays, managing chronic conditions, or living with age-related functional changeshelping them thrive safely and confidently at home.Key ResponsibilitiesConduct comprehensive in-home assessments to evaluate physical function, cognitive abilities, environmental safety, and personal goals.Develop and implement individualized treatment plans to promote independence in activities of daily living (ADLs), including dressing, bathing, mobility, and meal preparation.Recommend and fit assistive devices such as grab bars, transfer aids, walkers, and adaptive equipment.Provide caregiver and family education on safety, mobility, and fall prevention strategies.Collaborate with other healthcare professionals including nurses, physiotherapists, PSWs, and case managers to deliver coordinated care.Support clients with chronic disease management, rehabilitation post-surgery or injury, and mental wellness interventions.Document all assessments, care plans, and progress reports in compliance with regulatory and organizational standards.Assist with ADP applications and community resource referrals as needed.Participate in quality improvement initiatives and clinical team meetings.QualificationsRegistered and in good standing with the College of Occupational Therapists of Ontario (COTO)Minimum 2 years of clinical experience, preferably in community or home careADP authorizer status (Mobility) or eligibility to become authorizedStrong knowledge of assistive devices, functional rehab, and cognitive assessmentsExperience working with diverse populations, including seniors, individuals with disabilities, and those with chronic conditions or post-acute care needsExcellent interpersonal, problem-solving, and clinical documentation skillsProficiency with electronic health records and mobile documentation toolsValid Ontario drivers license and access to a reliable vehicle requiredMultilingual skills, particularly French, considered an assetWhat We OfferCompetitive salary and benefits packagePaid mileage and travel timeContinuing education support and mentorshipFlexible scheduling within a full-time modelStrong administrative and clinical support teamOpportunity to make a meaningful impact in the lives of clients and familiesJoin us in delivering high-quality, person-centered care where its needed mostat home.
