Job Title: Clinical Pharmacist Job Description We are seeking a skilled and compassionate clinical pharmacist to join our interdisciplinary team. As a clinical pharmacist, you will play a vital role in providing patient-focused care by completing medication reconciliations, maintaining patient medication profiles, attending patient care conferences, and collaborating with healthcare providers. The ideal candidate will have a strong understanding of drug therapy, pharmacology, and pharmaceutical sciences, as well as excellent communication and interpersonal skills. You will work closely with patients, families, and healthcare teams to develop personalized care plans and ensure seamless transitions of care. Responsibilities include: Providing clinical pharmacy services to improve patient outcomes by applying knowledge of pharmacokinetics, pharmacodynamics, and pharmacogenomics Ensuring continuous improvement in the delivery of prescribed treatments through effective monitoring and evaluation Identifying, preventing, and resolving medication-related problems by conducting thorough medication histories and developing tailored care plans Dispensing and distributing medication to patient care units while ensuring adequate stock levels for nursing staff Providing specialized pharmaceutical services to enhance therapeutic outcomes, including advising prescribers on issues concerning drug toxicity and side effects Collaborating with interprofessional teams, patients, and family members to optimize individualized care plans Maintaining compliance with professional standards and regulatory requirements, including those set by the Ontario College of Pharmacists and the National Association of Pharmacy Regulation Authorities Supervising pharmacy technicians and contributing to their annual performance reviews This is an exciting opportunity for a dedicated and detail-oriented clinical pharmacist to make a meaningful difference in the lives of our patients and contribute to our commitment to excellence.
