Position Summary As part of the Altum Health (AH) Team, the Integrated Clinical Lead (ICL) works in support of and reports to the Altum Health Site Manager. The ICL Lead provides leadership to the care team, oversight on the quality and effectiveness of care delivery and clinical consultation. Acting as a resource for staff, the ICL monitors efficiency and quality of clinical service delivery and provides guidance to improve the outcomes within their assigned program(s)/service(s). The ICL provide excellence in the patient experience by working closely with all members of the care team to create a one care team experience. The ICL improves the experience and satisfaction of the entire care team by facilitating communication and collaboration in support of an efficient and effective care journey. As a leader in a multidisciplinary care team, this role focuses on providing comprehensive care to support a patients overall health and well-being including patient-centered navigation, case management and coordination of services to patients as they access care within Altum Health. Duties Clinical leadership and collaboration with clinical and operational teams across the organization to support consistency in quality of service delivery and outcomes achieved Supporting and overseeing a patients care path through the various care settings to ensure continuity of care Implementation of initiatives to improve the effective and efficient delivery of services within assigned program(s)/service(s) Identification of opportunities for continuous quality improvement to support achievement of business and clinical performance targets Assisting in the development of resources and tools to support quality of service delivery and improved outcomes Acting as a liaison between the customer/referral source, community healthcare providers and clinical service delivery teams Maintaining strong relationships with internal and external stakeholders including involvement in business development activities as appropriate. Performs all duties in alignment with Altum Health and UHN strategic business objectives, vision, mission and values, and organizational policies and procedures. Qualifications At minimum a Masters Degree in a Regulated Health Profession or recognized equivalent, required. Prior clinical leadership experience preferred. Professional Affiliations/Memberships Registration with respective Legislative/Professional Regulatory College, required. Membership in Canadian/Provincial Professional Association, preferred. Leadership/Additional Skills Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance with UHN and Altum Health standards. Ability to develop, implement and monitor quality improvement initiatives. Comfortable making recommendations and decisions direction that are sound, well informed and objective with minimal direction. Ability to plan strategically, articulate a clear vision, direct and engage staff in its pursuit. Ability to empower staff to practice at their best. Exercises initiative and good judgment with ability to manage multiple competing priorities and deadlines. Knowledge of applicable legislative, UHN and/or Allied Health policies.
