This position is responsible to provide initial and ongoing assessments and financial reviews for Long Term Care (LTC) subsidization eligibility to residents requiring LTC (nursing level of care). Assessments are completed for residents waiting for LTC admission, acute care patients with a designation of Alternate Level of Care - Long Term Care (ALC-LTC) and annual reviews for all subsidized residents living in LTC. The incumbent works collaboratively with colleagues, applicants and their representatives to assess eligibility in a timely fashion by accurately determining the applicant''s net income and determining monthly accommodation payments for all subsidized residents. Duties: Responsible for understanding and clearly explaining the LTC subsidy program to all applicants Access and investigate requests for information and complaints regarding a subsidy application; response to applicants, families LTC and Acute Care staff regarding same Determine program eligibility of clients requesting LTC subsidy Complete assessment and send correspondence to appropriate parties (applicant, their designated contact, LTC home, and/or Acute Care financial staff) Conduct annual reviews for all subsidized residents living in a LTC home Create and distribute review documentation to all appropriate parties (client, appropriate LTC home, and Health PEI LTC staff designated to reimburse private LTC homes) Support the work of the Long-Term Care Admissions Committee (formerly known as the ''Placement Committee'') Provide designated committee with all appropriate information on clients pending LTC admission; and serve as a member of an LTC Admission Committee as required Conduct assessments for Alternate Level of Care Long Term Care for appropriate Acute Care Patients Other assigned duties that aid in the efficiency of the LTC Subsidization Program Minimum Qualifications: University degree preferably in Business, Social Services or a related field; This is a bilingual position. To qualify for this position a candidate must have excellent written skills and Advanced oral skills proficiency in both English and French A high degree of proficiency in typing, editing, and composing correspondence with word processing tools (Microsoft Word), working with (manipulating and creating) Spreadsheets and E-mail (Outlook); and Experience working with the geriatric population and or the general public. Must have considerable experience and efficiency in financial planning, preferably through completion of a formal program such as Certified Financial Planner Must have extensive computer knowledge including use of spreadsheets, word processing database programs and internet Must have extensive experience working with the public Excellent interpersonal and communication skills, time management, problem solving and organizational skills Must have ability to work independently with minimal supervision Valid driver''s license and access to a reliable vehicle is required Good previous work and attendance record Current knowledge of the Long-Term Care Subsidization Act and/or experience and knowledge of the people and services provided in long term care sector is preferred Other Qualifications Experience in undertaking independent projects of research and analysis with minimum direction and supervision; Knowledge of the Health System of Prince Edward Island would be an asset; Knowledge of the Long-Term Care Program''s function, priorities and policies, and of staff roles and relationships would be an asset; Skill in organization, time management, and interpersonal communication, both oral and written; Valid driver''s license and access to a reliable vehicle is required; and Ability to work under a demanding workload.
