Our organization is currently seeking a skilled Clinical Social Worker to join our team. About the Role The successful candidate will be responsible for providing counseling, case management, assessment, consultation and intervention to a broad range of patients and their families. The role involves working with individuals who present with complex social, family or marital problems and requires a high degree of autonomy within professional expertise. Key responsibilities include: Completing psychosocial assessments and developing case plans; Providing brief counseling services to patients and their families; Coordinating information and services, making targeted referrals, collaborating with other community services, professionals and programs and developing treatment plans for patients and their families; Responding to referrals from healthcare team members, patients and their families on situations requiring extensive assessment of the immediate and long-term needs of the patient and family; Acting as a case manager to coordinate patient services between the members of the healthcare team and community agencies programs and professionals; Providing stabilizing interventions for individuals and families in crisis; Providing direction to physicians/nursing about Adult Protection and Public Trustee matters and Provincial Guardianship; Providing direct intervention in mediation issues where there are incidents of patient or family conflict with the hospital/staff; Providing assistance in the management of bed crises and working with the Social Work Department and hospital Management to strategize and implement solutions; Providing an active liaison function with community-based social services agencies and groups in developing case plans for patients; Providing professional consultation and periodic in-service training to members of the healthcare team as appropriate in the general hospital and community; Being a responsible advocate for patients and families within the healthcare system, balancing the rights and needs of the person with the limitations of the system; Attending rounds as required; Representing the organization at provincial or regional meetings as required; Supervising students as required; Performing other duties as required. Required Skills and Qualifications To be successful in this role, you will require: A Master''s degree in Social Work; Considerable experience preferably in the healthcare sector; Eligibility for registration with the relevant professional registration board; Demonstrated ability and commitment to work effectively in an interdisciplinary team environment; Working knowledge of relevant legislation and regulations.
