Quality Improvement and Performance Lead Full-Time and Fixed-Term 13 Month Employee Contract (Possible Extension) The Alliance for Healthier Communities is the voice of a vibrant network of community-governed primary health care organizations. POSITION The Quality Improvement and Performance Lead (QIPL) collaborates with Community Health Centre (CHC) teams in a Learning Health System (LHS) environment to develop, coordinate and implement quality improvement, performance and change management activities and provide decision support. The QIPL will work in a collaborative reporting relationship guided by a Memorandum of Understanding between Host Sites, the EPIC (Equity-Performance-Improvement-Change) Committee, and the Alliance for Healthier Communities (Alliance). RESPONSIBILITIES Supports the CHC network to ensure patient experience leading to meaningful improvement in outcomes and reduce health disparities. Leverages data, analytics, Quality Improvement (QI) methodology and best practice evidence to enable CHCs to improve client care and service delivery. Collaborates with CHC Networks, teams and providers (through Learning Collaboratives and/or Communities of Practice) to provide support in all five phases of the LHS learning cycle. Guided by the EPIC workplan, supports and enhances performance and information management initiatives, including Practice Profiles, panel size and SAMI/complexity. Contributes to Alliance member committees (e.g. EPIC, Regional User Groups). Performs knowledge translation functions for provincial initiatives such as OHRS, MSAA and QIPs. Develops working relationships with colleagues at CHCs to understand the complexity of CHC service delivery. QUALIFICATIONS Requirements Masters level (preferred) or University degree and experience in a related discipline (e.g. health administration, health informatics, health sciences, statistics, epidemiology, quality improvement). At least 5 years of experience in a healthcare environment in a decision support, quality improvement and/or change management capacity. Experience collaborating with multiple stakeholders and committees and translating knowledge for various audiences. Experience with Quality Improvement models/approaches, e.g. LEAN/Six Sigma, Model for Improvement. Working knowledge of coaching, change management principles and practices. Strong project management skills, including prioritizing and managing multiple projects with complex demands and short timelines. Proficient computer skills for report and presentation preparation, data analytics (e.g. advanced Excel, SPSS, Tableau), BI/COGNOS preferred. Proficient with Windows and Microsoft Office software applications including Outlook, Word, Excel, PowerPoint. Ability to conduct work in keeping with the Alliances commitment to anti-oppression, anti-racism (including anti-Black and anti-Indigenous racism), French language services, and to being 2SLGBTQ+ positive. Must possess a valid Ontario G drivers license to travel within Ontario. Bilingualism in both English and French in asset. WORK LOCATION This position will have some flexibility of working from home and will be required to be on-site when needed at the Alliance office in Toronto, Ontario, Canada. COMPENSATION Annual Salary:starts at$79,058 The Alliance offers an exceptional benefits package, including pension with HOOPP, extended health and dental benefits, and a generous vacation package.
