In this community nursing role, as a Visiting Nurse, you will work both on-call overnights and have scheduled visits to care for patients in various settings (e.g., home and school). You will work with patients with differing complexity of care needs and play a key role in helping patients remain safe and comfortable in their community for as long as possible. Execute and continuously monitor care plans Engage patients, families, and team members, using a collaborative approach by building rapport and trust What you bring: Registration with the CNO as a Nurse in good standing Valid drivers license and access to a vehicle is required Experience in delivering community care is an asset Reliability and flexibility Willingness to travel to and from patient residences/school settings and day programs ParaMed Home Health Care is Canadas leading provider of home care services.
