Full Time Pharmacist Position posting number: 250594 Rate of Pay: Reference Paramed Collective Agreement Schedule: Full Time Assignment Location: Walkerton, Kincardine Applications are currently being accepted for a Full Time Pharmacist for the Walkerton site of the South Bruce Grey Health Centre, with shifts also scheduled at the Kincardine site as required. Reporting to the Manager of Pharmacy, the Pharmacist will be responsible for medication distribution and clinical activities related to patients at SBGHC sites. This includes monitoring patient drug therapy and providing drug education to patients and other health care providers. The Pharmacist reviews all physicians orders, screening for drug interactions, therapeutic duplicates, dose range checking, therapeutic appropriateness and any other pharmacy related concerns. You will be involved in interacting with other interprofessional team members and patients to determine the most appropriate and effective course of treatment for patients, and implement interventions within the standards of practice for the discipline. On-call or evenings/nights/weekend coverage may be required in the future. Applicants must meet the following criteria: Required Graduate from an accredited post-secondary Pharmacy Program (i.e. B.Sc Phm, M.ScPhm, PharmD) Current registration (Part A) in good standing with the Ontario College of Pharmacists Ability to provide and promote patient centered care Leadership, problem-solving, decision-making and organizational skills Interviewing and patient assessment skills Proven verbal and written communication skills Interest and experience in patient education and health promotion Commitment to continuous professional development and continuing education General computer skills Travel between sites required; Possession of a valid Ontario Class G Drivers License and personal vehicle Ability to provide multi-site coverage Recent satisfactory performance and attendance records Advocates and supports a culture of patient safety and demonstrates an understanding of how the department service and assigned responsibilities contribute to overall patient safety at SBGBC through patient safety knowledge, skills and attitudes Preferred Membership in Canadian Society of Hospital Pharmacists Knowledge of Cerner information systems A General Hospital Residency Certificate or 3 years experience in a hospital with clinical pharmacist responsibilities To be considered for this position please submit resumes to Human Resources by email to clearly indicating the posting number noted above.
