VCH is looking for a Physiotherapist to join the Rehabilitation Services team for the Sunshine Coast Home Care Services, including Pender Harbour, Gibsons and Sechelt in BC. As a Physiotherapist with VCH you will be part of an interprofessional team delivering client- and family-centered care to adults with acute, episodic and chronic conditions in a community setting. Responsibilities Work within a client and family model of care with a variety of acute episodic and chronic disease adult populations and within the vision and values of the organization. In partnership with the client and family, establish safe, realistic goals and manage physiotherapy and related services in an interprofessional team model. Assess service eligibility, develop and implement care plans, deliver direct care, and provide care management to help clients navigate community services. Collaborate with system partners and integrate current evidence into practice. Qualifications Current full registration as a Physiotherapist with the College of Health and Care Professionals of BC (CHCPBC). Two (2) years recent, related experience in home & community care. Valid BC Drivers License & access to a personal vehicle for local travel. Knowledge, Skills & Abilities Basic knowledge of evidence-based clinical practice related to the designated clinical area. Broad knowledge of physical therapy standardized tests and measures. Broad knowledge of CHCPBC standards of practice and guidelines for physical therapists. Knowledge of patient-centered care and ability to work with others in a team and with clients and families. Ability to communicate effectively with co-workers and clients, both one-on-one and in groups; ability to work independently and with others; ability to provide effective consultation. Ability to establish workload priorities in a complex health care environment; recognize when referral to another health care professional is required. Physical ability to perform the duties of the position; skilled in the use of equipment and treatment techniques; basic computer literacy.
