Overview Coordinates, screens and prioritizes hospice palliative client activities by assessing clients, consulting with the interdisciplinary team and coordinating client care across the continuum of care; acts as a clinical resource to other nurses and team members by providing clinical guidance and/or nursing consultation; implements, evaluates and recommends clinical education plans for nurses and other staff in the designated clinical area; provides nursing and clinical education/training including assessment of clinical skills and training of nursing staff; reviews and evaluates care plans and takes corrective action. Responsibilities Supports the interdisciplinary team by monitoring the standards of care provided to clients and acts as a clinical nurse resource to staff; assesses, demonstrates and directs the provision of care; screens, assesses and prioritizes hospice palliative referrals; role models, coaches and/or mentors the interdisciplinary team in therapeutic communication with clients, family members and caregivers. Coordinates and prioritizes client care activities by providing formal clinical consultations to nurses, interdisciplinary team members and students; assesses client needs and progress, troubleshoots and provides clinical guidance in care management; consults with staff regarding referral options to other healthcare professionals. Consults with the interdisciplinary team and other healthcare providers to resolve client care issues; provides feedback to support critical thinking and responds to variances from guidelines; works with clients and families to minimize variances. Coordinates development and implementation of clinical education, training and/or orientation for the interdisciplinary team; identifies learning requirements and measures learning outcomes; implements teaching strategies aligned with objectives. Assesses individual and interdisciplinary team clinical skills; designs, develops, implements and evaluates clinical education programs in hospice palliative care; provides training for new team members and ongoing continuing education. Act as a preceptor, clinical resource and mentor by providing evidence-based teaching; collaborates with management to identify skill deficiencies and ensures staff competencies through training and coaching. Participates in quality improvement and risk management activities; evaluates changes to client care practice, reviews incidents, conducts safety audits and recommends policy/procedure changes. Participates in interdisciplinary rounds and care conferences; acts as advocate for the client to protect dignity, autonomy, respect, privacy and access to information. Assists with staff selection and performance evaluations; contributes to orientation and training plans and provides feedback to management for follow-up actions. Promotes the use of research in nursing; applies current research findings to practice to keep program content current and relevant. Sets measurable goals and maintains clinical competence through professional development; collaborates with the Manager to monitor progress and ensure timelines are met. Performs other related duties as assigned. Qualifications Education and Experience Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). At least three (3) years of recent related clinical nursing experience in the designated clinical area, including one (1) year in a supervisory/leadership role, or an equivalent combination of education, training, and experience. Completion of an applicable Canadian Nursing Association (CNA) certificate in the designated clinical area. Valid BC Driver's License and access to a personal vehicle for business-related purposes. Skills and Abilities Comprehensive knowledge of nursing theory and evidence-based hospice palliative best practices. Understanding of adult learning principles and critical thinking skills. Ability to develop, implement and revise education/training and orientation programs. Strong leadership, coaching and mentoring abilities. Ability to instruct others and facilitate learning of staff and/or clients/families. Effective verbal and written communication with clients, families, staff and other service providers. Ability to work independently and as part of an interdisciplinary team. Ability to build rapport with staff, clients and stakeholders; problem-solving and conflict management skills. Ability to plan, organize and prioritize work in a dynamic environment; operate related equipment and software. Physical ability to perform duties.
