Physiotherapist - Elgin County Area (Adults) VHA Home HealthCare is seeking a Physiotherapist for the Elgin County Area (Adults). The role provides physiotherapy consultation, assessment and planned interventions for diverse client groups across Southern Ontario and the Capital region, referred through VHA Rehab Solutions and Champlain Rehab Solutions. Responsibilities Assess clients using interviews, standardized and informal assessment techniques per professional guidelines. Implement client interventions based on assessment, client input, and integrated care plan. Ensure services are best practice, evidence-based, and evaluated with valid outcome measures. Attend client-related meetings to report findings, plans, progress and discharge as required. Coordinate delivery of care through ongoing communication with the clients team. Document assessment findings, treatment plans, progress and discharge summaries as required by the profession and the organization. Prepare statistical records of client care and related activities as required by VHA, the Ministry of Health and other agencies. Adhere to the code of ethics and standards of practice as defined by the Ontario College of Physiotherapists. Required Qualifications Baccalaureate or Masters degree in Physiotherapy from a recognized school. Registered, in good standing, with the College of Physiotherapists of Ontario. Minimum of 2 years clinical experience, preferably in the community. Specialized courses to support areas of clinical expertise. Clinical expertise to provide a variety of physiotherapy interventions. Knowledge of community care requirements and resources. Excellent consultation and teaching skills, and uses an evidence base in practice. Familiar with the routine use of valid and reliable outcome measures to support clinical interventions.
