Clinical Pharmacy Specialist - Eagle Ridge Hospital Position: Part-time Clinical Pharmacy Specialist in Emergency Location: Eagle Ridge Hospital, Port Moody, BC Salary: CAD $53.31 - $66.63 per hour Overview Join Fraser Health Authority as a Clinical Pharmacy Specialist in the Emergency department. This part-time opportunity focuses on delivering advanced pharmacotherapy expertise, education, and research within the designated specialty area. Responsibilities Provide consultation services to internal and external health care professionals and apply advanced theory to assess, plan, implement and evaluate the delivery of pharmaceutical care in the designated specialty area. Provide clinical direction on complex cases, including evidence-based evaluations of drug therapies and patient medical histories; participate in patient education, discharge consultations, and policy/procedure planning. Assist in developing drug therapy guidelines for individual disease conditions; identify and initiate clinical drug research; prepare case studies and articles for professional journals. Conduct pharmacological education or in-service sessions; represent the department on multidisciplinary care committees and at external events. Ensure QA programs for the designated area, conduct audits, and implement corrective actions as required; contribute to quality improvement activities and program modifications as needed. Provide academic teaching to undergraduate/graduate students and pharmacists; participate in Medication Use Evaluation programs and other research initiatives. Qualifications Current registration as a Pharmacist with the College of Pharmacists of British Columbia. Completion of an accredited Hospital Pharmacy Residency Program (Certificate of Year 1 and Year 2) as supplemented by two years of recent relevant hospital pharmacy experience, including one year in the designated specialty area, or an equivalent combination of education, training and experience. Designated Specialty Areas (examples): Anticoagulation, Cardiology, Critical Care (Adults/Neonates/Pediatrics), Emergency, Infectious Diseases, Internal Medicine, Palliative/End of Life Care, Nephrology, Neurology, Oncology, and others as listed. Skills and Abilities Strong communication skills, both verbal and written, with ability to present to diverse stakeholders. Ability to establish and maintain effective working relationships with health practitioners, patients, families, and the public. Capability to coach, mentor, teach, and facilitate learning in a multidisciplinary setting; develop curricula and evaluate clinical rotations. Independent judgment to recommend, implement, monitor and adjust pharmacotherapeutic plans based on patient-specific information. Proficiency in literature retrieval and evaluation to support clinical care pathways and medication use evaluations. Project leadership, collaboration with teams of pharmacists and students, and experience in clinical research design and execution. Physical ability to perform duties as required.
