Responsibilities Provides physiotherapy assessment of the patient's physical status, functional abilities, needs, and goals by reviewing records, interviewing the patient, and coordinating with the health care team; performs examinations and monitors health to improve or maintain physical performance. Analyzes patient needs, establishes diagnosis and prognosis, and develops an evidence-based intervention strategy with short and long-term goals; discusses findings with patient and caregivers and selects appropriate intervention models. Implements interventions according to treatment guidelines and educates patients and caregivers on self-management and prevention. Participates in interprofessional care planning, updating the care plan and ensuring effective implementation and communication for care/discharge planning. Evaluates and documents effectiveness of interventions, monitors health status and functional outcomes, and adjusts plans to achieve optimal independence. Identifies appropriate referrals, screens for priority of intervention, and facilitates referrals to health care/community programs. Collaborates with patients and families in developing and modifying the care plan; maintains a therapeutic relationship with respect for privacy and autonomy. Maintains patient records, documents progress, and compiles statistics as requested. Delegates components of treatment to rehabilitation assistants with clear instructions and ensures quality patient care. Acts as a physiotherapy resource to team members and participates in orientation and in-services as needed. Participates in team and program activities to enhance care delivery and promote evidence-based practice; contributes to quality and safety improvements. May participate in approved research, special projects, and committees as appropriate. Performs other related duties as assigned. Qualifications Education and Experience Current full registration with the College of Health and Care Professionals of BC. Skills And Abilities Knowledge of physiotherapy theory and practice within a patient- and family-centered model of care. Knowledge of CHCP BC standards of practice and guidelines for clinical practitioners. Knowledge of physiotherapy standardized tests, measures, equipment and related techniques. Broad knowledge of other health care professions and their role in patient care. Effective communication with patients, caregivers, and interprofessional team members (verbal and written). Ability to teach and provide effective consultation to patients, caregivers, and health care professionals. Ability to work independently and in collaboration with others. Ability to establish workload priorities and adapt to unexpected events. Basic knowledge of research processes and methodology. Ability to operate related equipment and software. Physical ability to perform the duties of the position.
