Overview In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care to patients in an acute medical unit, in collaboration with the interprofessional care team. Responsibilities Assess patients and collect information from a variety of sources using observation, communication, data review and physical assessment. Make nursing diagnoses and plan, perform and evaluate nursing interventions and care. Collaborate with patients and families in developing and modifying the care plan; establish a therapeutic relationship, advocate for patient rights, privacy, dignity and access to information, and support patient autonomy. Participate in discharge planning and care coordination with the interprofessional care team and Patient Care Coordinator. Coordinate nursing care, set priorities, delegate tasks to support staff, and make referrals according to guidelines and standards. Collaborate with physicians, LPNs, educators, and other health professionals to identify and resolve issues and adjust care plans as needed. Educate patients and families about illness prevention, health restoration and health maintenance; assess learning needs and support self-care and discharge planning. Discuss referrals to appropriate health care or community services with patients and families. Document assessments, diagnoses, interventions, outcomes and referrals according to practice standards and Fraser Health procedures. Participate in nursing committees, care conferences and quality improvement activities; engage in ongoing education and maintain clinical competence. Act as a preceptor or mentor to new nursing staff and students; provide feedback to leadership on care programs and tools. Perform other related duties as required. Qualifications Education, Training & Experience Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Completion of Cardiology components and post-basic courses such as Critical Care or High Acuity, or an equivalent combination of education, training and experience. Current Basic Rescuer Certificate. Skills And Abilities Clinical proficiency in the designated clinical area with strong assessment and nursing intervention skills. Effective communication with patients, families, and interprofessional care teams using verbal and written methods. Ability to provide person-centered care with empathy and respect for diverse cultures and preferences. Ability to analyze data from multiple sources to solve problems and make informed decisions. Ability to teach patients and families about health care topics and self-care. Organizational skills to prioritize work and collaborate with team members. Ability to use evidence-based practice and relevant equipment and computer applications. Physical ability to perform the duties of the position.
