Vancouver Coastal Health is looking for a Registered Nurse (RN) to join the Primary Care Team in Sechelt, BC. Relocation assistance and staff temporary housing may also be available. As a Nurse with the Primary Care Team, you will provide nursing care in collaboration with the Primary Care Provider and the PCN Interdisciplinary team. Responsibilities Collaborate with the Primary Care Provider (Family Physician or Nurse Practitioner) and other PCN Interdisciplinary team members to assess, plan, implement, and evaluate nursing care for patients/clients and caregiver(s). Maintain partnership with the Primary Care Provider according to the local PCN care delivery model co-designed by PCN Partners (local Division of Family Practice, VCH Community of Care and First Nations Partners). Apply health promotion, chronic disease management, advance care planning, and self-care management principles to conduct assessments, identify problems, provide direct care, and address variances from the plan of care. Provide continuity of care by liaising with team members, coordinating ongoing care (internal and external), supporting transitions and navigating services. Act as a nursing resource for the team by applying problem-solving and critical thinking skills and providing information based on nursing theory and practices. Education & Experience Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One (1) year recent, related clinical experience relevant to the designated clinical area, or an equivalent combination of education, training, and experience. Valid B.C. drivers license and access to a vehicle. Knowledge, Skills & Abilities Critical thinking skills within safe limits of patient care. Understanding of primary care theory and practice within a patient/client and family centred model of care. Knowledge of relevant Professional Regulations and Standards (e.g., CHNC competencies and standards of practice). Evidence-based clinical practice related to primary care; knowledge of provincial acts, regulations, and program policies for home, community and primary care. Ability to provide culturally safe care across diverse populations and promote Indigenous Cultural Safety. Trauma-Informed Practice, Harm Reduction and Recovery Oriented Care; ability to recognize when referral to another health professional is required; ability to support seamless transitions in care. Effective communication, conflict resolution, independent work skills, and ability to set priorities for care. Knowledge of other health disciplines and their roles in patient care; ability to use research in practice; teaching and coaching abilities for patients, families and caregivers. Computer skills, including use of a computerized patient care information system; CPR proficiency; physical ability to perform duties and operate related equipment.
