Job Description

Overview Skin Wellness Associate Nurse - SWAN (RPN) role at SE Health. Full-Time or Part-Time. Offering a $3,000 Sign on Bonus. Position Summary: As a member of the Wound Ostomy Continence (WOC) team, the Skin Wellness Associate Nurse (SWAN) works collaboratively with the Nurse Specialized in Wound, Ostomy and Continence (NSWOC) to meet the needs of individuals/families with ostomies, acute and chronic wounds, and urinary and fecal continence concerns. Responsibilities Clinical Support and Consultation: Perform detailed holistic assessment of clients with wound, ostomy and/or continence related concerns; formulate treatment recommendations in collaboration with the interprofessional team; develop a plan of care; evaluate the plan of care and escalate to NSWOC when clinically indicated; involve K2P Wound Care Resource Nurses in the evaluation process as appropriate; provide education and mentorship to build wound, ostomy and continence knowledge; provide consultation to care providers; facilitate dissemination of wound, ostomy, and continence knowledge; coordinate/collaborate with interprofessional team across the continuum of care. Program Development/Leadership: Support quality health care through teaching, mentoring, and quality monitoring; optimize client outcomes through review of quality improvement reports; identify nursing trends related to wound ostomy and continence management; assist in development of information tools and educational initiatives; remain up to date on current evidence; collaborate with professional nursing associations; participate in local funder initiatives/working groups as required. Qualifications & Skills: Current registration with the College of Nurses of Ontario; successful completion of the SWAN program; flexibility and a commitment to advancing wound, ostomy, and continence care; willingness to participate in further education; strong interpersonal and communication skills; ability to work independently and in a team; knowledge of quality improvement processes; CPR certificate; valid drivers license and vehicle; updated immunization records and clear background checks; willingness to attain SE Health certifications; availability to work days, evenings, nights and every other weekend; strong computer skills; physical ability to perform required tasks.
