The Work You Will Do Collaborate with Community Health Nurses in clinics to provide support Assess patients and develop individualized care plans Follow up and reach out to community members. Assist patients In some cases, you may be the sole MHN in the community, so it is essential to be comfortable working independently with limited resources Why Bayshore?
