Key Responsibilities Provide comprehensive patient care withinthe NP scope of practice, including assessment, diagnosis, treatment,prescribing, and health promotion. Deliver services in clinical,community, institutional, or remote settings as required by theClient. Support both acute and chronicdisease management , preventive health care, and patient/familyeducation. Collaborate with physicians, nurses, alliedhealth professionals, and administrative staff. Maintain accurate patient records incompliance with PHIPA (Personal Health Information Protection Act) andClient policies. Adapt service delivery based on Client needs in-person or hybrid (initial in-person with virtual follow-ups),when permitted . Participate in multidisciplinary care teamsand support continuity of care across service settings. Ensure compliance with AODA, OHSA,FIPPA, and all applicable Ontario healthcare regulations . Requirements Qualifications & Requirements Current registration as a NursePractitioner (NP) with the College of Nurses of Ontario (CNO) ingood standing. Minimum 2 years of clinical NPexperience (primary care, hospital, community health, orspecialty practice). Demonstrated ability to work independentlyand collaboratively in diverse healthcare environments. Strong communication, assessment, anddecision-making skills. Ability to work across multipleOntario regions , with willingness to travel or relocate for specificassignments as required.
