Join our team as a Nurse Practitioner at the Williams Lake Primary Care Hub (WLPCH), where you can leverage your clinical expertise and passion for providing high-quality, compassionate care to make a meaningful impact in the community. The WLPCH is dedicated to delivering team-based, integrated primary care to both attached and unattached patients in the Williams Lake area, with the added opportunity to provide monthly outreach to the Tilhqotin and Secwpemc Nation communities. Our clinic offers a hybrid care model that includes in-person, in-reach, and comprehensive virtual appointments, ensuring patients and families have access to high-quality, patient-centered care. You will collaborate with a professional team committed to providing culturally safe, trauma-informed care that prioritizes the needs of individuals and families. In this role, you will work closely with patients, clients, residents, and other healthcare professionals to identify emerging health trends and develop evidence-based strategies that enhance patient care and improve community health outcomes. You will also contribute to the development of health policies that shape local practices and services. Regular participation in peer and self-review processes will allow for continuous evaluation and improvement of the care provided at both the individual and population levels. Seeking 3.0 FTE of Family Nurse Practitioner to provide primary care. The WLPCH will operate 6 days per week, year round, 0800-2000 (variable schedules for extended coverage). Interior Health establishes salaries within the minimum and maximum of the salary range based on consideration of the qualifications, experience of the applicant. Key Responsibilities Diagnosis and Treatment: Assess and diagnose undifferentiated diseases and conditions, provide treatment, and prescribe medications in line with the Nurse Practitioners scope of practice and regulatory standards. Care Management: Monitor ongoing care, order diagnostic investigations, interpret results, and develop tailored treatment plans to optimize patient outcomes. Patient Communication: Establish care priorities, provide follow-up treatment, and communicate effectively with patients and families about diagnoses, care plans, and prognoses. Collaboration and Consultation: Work closely with physicians and other healthcare professionals to assess, diagnose, and create individualized treatment plans. May also admit and discharge patients as per organizational policies. Prescribing Medication: Prescribe medications within legal and regulatory guidelines, ensuring the safe and effective use of pharmaceutical treatments. Leadership and Supervision: Lead and guide other nursing and healthcare personnel. Provide education, supervision, and performance evaluations to support professional development. Research and Innovation: Contribute to research aimed at enhancing patient care and advancing nursing practices. Participate in initiatives to influence healthcare policies and population health strategies. Health Promotion: Lead health promotion activities and screenings, focusing on at-risk populations to prevent illness and promote well-being. Education and Mentorship: Facilitate interdisciplinary education through case presentations, mentoring, and collaboration in clinical and community settings. Support continuous learning and knowledge sharing. Policy Development: Develop, implement, and evaluate policies related to nursing, interdisciplinary care, and health system practices. Palliative and Long-Term Care: Serve as the most responsible provider for patients in palliative, hospice, and long-term care settings when required. Safety and Compliance: Adhere to Occupational Health and Safety guidelines to maintain a safe and healthy work environment. Ensure compliance with training and incident investigation protocols. Additional Responsibilities: Perform other duties as necessary to support patient care and organizational goals.
