These programs help ensure clients receive timely support for a well-organized transition from hospital to home, with clinical services adjusted directly in the clients home based on fluctuating need. By embedding multidisciplinary teams at discharge from hospital, weve reduced hospital readmissions and accelerated recovery times. Together, these programs signal VHAs leadership in delivering safe, seamless new models of care from hospital to home. We are looking for a motivated Occupational Therapist to support these programs whose values align with the values of these programs- client centered practice, working within a team model in community to support most vulnerable clients, responsiveness and flexibility to meet clients where they are in their health care journey, ability to respond to time sensitive requests and skilled communication with clients and care team. VHA's Community Access Occupational Therapist is part of an innovative model of client care contributing to the rapid increase in home care services for the aging population. This is a key team member to ensure that clients from our hospital-to-home programs have access to OT services in a timely manner. The OT will be responsible for seeing clients at home a few days after they are discharged home to start their intervention, following up on goals set up by the hospital to keep clients safe at home while improving their functional abilities. This role has some administrative and scheduling support so you can focus on direct client care.**Key Areas of Accountability**VHA's Community Access Occupational Therapist have the following key areas of accountability, but are not limited to:* Assesses and implements Occupational Therapy Interventions in a community setting in accordance with professional practice guidelines and standards established by VHA and the perspective regulatory college.* Assesses clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA Rehab Solutions.* Interventions include but are not limited to: Home safety equipment and mobility aids, falls preventions and energy conservation strategies, palliative care, ADL Training, cognitive remediation/compensatory strategies, and linking to community resources/funding.* Attend client related meetings to report on assessment findings and discuss plans for interventions, progress reports and discharge as required.* Ensure coordinated delivery of care through ongoing communication with all members of the clients team regarding plans and schedules for interventions.* Document all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the agency.* Ensure that services provided are best practice, evidence based and evaluated through the routine use of valid and reliable outcome measurement.* Adheres to the code of ethics of the profession, in maintaining standards of practice for the profession as defined by the College of Occupational Therapist of Ontario.**Required Qualifications:*** Degree in Occupational Therapy from a recognized school of Occupational Therapy.* Member in good standing with the College of Occupational Therapists of Ontario.* Membership in professional association preferred.* Experience within the professional discipline is an asset.* Specialized courses to support areas of clinical expertise.* Ability to work independently.* Familiar with the routine use of valid framework and reliable outcome measures to support clinical interventions.* Demonstrated commitment to client and family centered care.* Demonstrated ability to recognize safety concerns, communicate effectively using a computer and/or mobile device* Benefits and pension plan for permanent eligible employees* Compensation for education and professional development* VHA is a leading learning organization so we can provide you with comprehensive orientation and training at the start and throughout your career* Mentorship and peer support* Career development opportunities* Employee and family assistance program* Wellness resources* Perks & discounts* Staff & service provider events**Accommodation and VHAs commitment to DEI**At VHA Home HealthCare (VHA), we passionately strive to uphold our commitments to being an Equal Opportunity Employer.
