Responsibilities Individualized pharmacotherapy and counselling patients about their medication therapy. Work in collaboration with physicians and other health care providers to develop, implement and monitor therapeutic plans. Assist in the development of drug therapy guidelines for disease conditions. Prepare and submit for publication in professional journals; contribute case studies and articles sharing new knowledge. Act as department resource person within the assigned area; participate in medication use evaluations, quality improvement initiatives and clinical drug trials. Provide mentorship and supervision to pharmacy students, residents and new staff. Ensure prescriptions are validated and dispensed accurately; address medication safety concerns and resolve discrepancies with the health care team; document professional practice activities proficiently. Detailed Overview The Clinical Pharmacist provides clinical pharmacy services including individualization of patient medication therapy, patient medication counselling, and collaboration with the health care team to discuss therapeutic plans and guidelines. Provides support to the Pharmacy & Therapeutics Committee, contributes manuscripts for journals, and participates in medication use evaluations and clinical drug trials. Performs drug distribution duties and ensures safe, accurate dispensing and documentation. Responsibilities Consult with physicians, nurses and other health care providers to develop therapeutic plans; document in the patient health record per regulations and standards. Individualize therapy by reviewing literature, patient data, and laboratory results; adjust plans to meet patient needs. Provide patient medication counselling and explain plans to patients and families. Assist in developing drug therapy guidelines and monitor outcomes. Participate in ward rounds and unit-level pharmacy services to enhance patient care. Serve as department resource, provide detailed drug information, and support staff and students. Contribute to case studies and articles for publication; supervise students and residents; participate in quality improvement activities. Perform drug distribution duties and ensure safe dispensing; communicate with prescribers to clarify and resolve issues; document interactions. Maintain narcotic and controlled drug records; participate in medication safety initiatives and quality control. Qualifications Education and Experience Current registration as a Pharmacist with the College of Pharmacists of British Columbia. Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience. Skills and Abilities Knowledge of clinical pharmacy practice, medication use evaluation, research processes, and hospital drug distribution. Ability to apply clinical knowledge with minimal supervision; strong communication and relationship-building skills.
