ROLE PURPOSE The Home Share Program is a residential option in which an adult with a developmental disability lives with a caregiver who is contracted with Connective to provide ongoing daily support. The Home Share Provider role is to provide a safe home environment where adults can develop social and life skills to ready themselves to reach their full potential for independence. The Home Share Provider applying for this posting will be required to move into the individuals existing home in the Skeena Street area in Vancouver. This individual must have access to support at all times and can only be left alone for a maximum of one-hour at a time. Respite support may be available. The individual has a bird pet and is open to dogs. KEY ACCOUNTABILITIES Reports to Home Share Manager on a regular basis and in bi-monthly check-ins Provides flexible and evolving support to individuals changing needs within a home Works with the guidelines and instructions within the Home Sharing Services Handbook and all other policies and standards set for the Home Sharing Program Ensures a stable, structured and home-like environment and positive and positive role models to guarantee the safety of the Individual Takes appropriate action to accomplish the objectives set for the service user Cooperates with any plan established by the Agency or the Ministry for the placement of the Individual outside the Home Establishes safety and emergency procedures to provide support, care and supervision consistent with the Personal Service Plan of the Individual Fulfills necessary administrative duties, such as completing Critical Incident Reports or Missing Persons Reports Ensures that the Individual and their chosen family are involved in all planning and decision-making on matters affecting the Individual, whenever possible Assists the Individual in making effective choices concerning the development and maintenance of friendships and relationships Conducts regular and ongoing communication with the Individual's support teams, maintaining clear and professional relationships EXPERIENCE AND SKILLS We are seeking a combination of specific skillsets for this role. The ideal Home Share Provider may: Have experience supporting individuals with disabilities Be able to provide personal care, including support with toileting Be adept at conflict resolution, redirection, and de-escalation Be familiar with Positive Behaviour Interventions and Supports Have experience in providing care to individuals facing multiple barriers Be proficient in executing person-centered approaches Have a gentle approach when implementing firm boundaries REQUIREMENTS Willing to re-locate to the Skeena Street area in Vancouver Ability to successfully complete a Criminal Records Check Current Emergency First Aid Certificate Complete the Home Share Provider screening Complete MANDT Training, and other Home Share specific training, provided by the agency Be consistent in implementing firm professional boundaries with persons served Class 5 Driver's License and a vehicle are assets PAYMENT This is a contractor position.
