This role offers the flexibility to set your own schedule and travel within the Newmarket area and surrounding regions, with location coverage tailored during the interview process. As an OT at CBI Home Health, youll be part of an interdisciplinary team that uses a holistic approach to service delivery. Youll help clients achieve their goals through face-to-face visits or digital health platforms. This will be an independent contracted role with up to 8 hours per week working in a facility-based hospital transition care unit in the York-Simcoe region. ADP Authorizer status for seating and mobility is mandatory (or willing to acquire). Additional community hours in an interdisciplinary integrated care model can be discussed as available and for the right candidate. What Youll Do (Responsibilities) Assessment & Implementation: Provide OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications. Team Collaboration: Engage in a collaborative, interdisciplinary approach to care and services. Holistic Care: Support a restorative health approach, offering interventions, health coaching, and self-management support. Digital Health: Utilize our digital platforms for care delivery, collaboration, documentation, and intervention. What You Need To Succeed (Qualifications) Professional Standing: You are a registered member in good standing with the College of Occupational Therapists of Ontario. ADP Authorizer: Mandatory. Mobility: Ability to travel within the Newmarket area (valid drivers license and insurance required). Client Focused: You embrace person- and family-centered care with passion and empathy.
