Physiotherapist - Calgary CBI Home Health Overview CBI Home Health is hiring Home and Community Physiotherapists (PT) who are excited by flexible hours and the freedom to create their own schedule. This role requires travel within the Calgary area. As a PT, you will be part of an interdisciplinary team using an integrated and collaborative approach to service delivery, working together to effectively meet clients goals. The In-Home Restorative Care Program provides personcentered care at home for individuals who have recently experienced a loss of functional ability. The goal is to increase strength, mobility, and functional ability so clients can remain at home as long as possible. Clients receive medical and nursing oversight, active rehabilitation programming, and access to social work and personal care support as needed. Care plans are personalized, goaloriented, and aligned with the AHS Rehabilitation Model of Care. Restorative care is a timelimited, multidisciplinary, goaloriented, and clientcentered approach that helps medically stable clients regain or retain independence and avoid higher levels of health care. Services are delivered by an integrated team that includes a case manager, regulated health professionals such as OT, PT, SW, a Nurse Practitioner, and rehab assistants with expertise in working with the frail elderly population and/or adults living with complex chronic conditions. Responsibilities Work with clients to develop and implement individualized treatment plans focused on maintaining and enhancing physical abilities and wellbeing. Manage and prioritize clinical caseloads, coordinating and delivering therapeutic services. Participate in a collaborative care model providing holistic, comprehensive care. Promote positive relationships between clients, team members, funders, and the community. Qualifications Registered as a member in good standing with the College of Physiotherapists of Alberta. Ability to travel within the Calgary area (valid drivers license and reliable car required). Level C CPR certification issued within the last 12 months. Emphasis on person and familycentered care. Passionate, empathetic, with good judgement and critical thinking for client decisions. Excellent communication skills, able to collaborate within an interdisciplinary team and work independently. Values the role of a therapist in keeping clients at home when that is of personal importance.
