Job Summary The PSW Coordinator oversees, supports, and monitors Personal Support Workers (PSWs) to ensure direct personal and nursing care fulfills residents holistic needs in accordance with Southbridge policies and procedures. The role also assumes responsibility for daily operations of PSWs, including ensuring tools, equipment, and resources are available on all shifts. Report To: Director of Care Qualifications Personal Support Worker (PSW) Program Certificate or Diploma meeting the appropriate standards established by the Ministry of Training, Colleges and Universities, the National Association of Career Colleges, or the Ontario Community Support Association. Current enrollment in a nursing program (with at least one year completed) is acceptable. Knowledge and experience in care planning systems and required documentation. Excellent verbal and written communication skills with staff, residents, and families. Demonstrated ability to work independently with strong leadership and teamplayer skills. Working knowledge of WHMIS, Basic Workplace Safety Training, mandatory reporting, First Aid and CPR, and training in Gentle Persuasive Approach is an asset. Current Vulnerable Sector Police Check and uptodate immunization and vaccination records. Demonstrated record of good attendance. Experience in longterm care is required. Responsibilities Under the direction of the Director of Care, ensure FLTCA and Canadian Council on Health Services Accreditation standards are met. Observe, report, and record resident care electronically in compliance with policies and procedures. Provide input into the development and implementation of policies and procedures reflecting the homes mission, vision, and values. Orient and mentor new PSWs as required. Collaborate with registered nursing staff to develop appropriate care plans for each resident. Ensure PSWs follow established care plans. Participate in 100% of all required education and training programming. Provide education and training to PSWs, including safe lifts and transfers. Maintain professional communication with residents, families, and team members. Complete all scheduled audits and assigned reports. Ensure PSWs have the necessary tools and supplies to perform their duties. Participate in IPAC requirements and observe IPAC practices by PSWs. Participate in the Health & Safety Team. Conduct PSW interviews, hiring processes, and performance management. Participate in continuous quality improvement and risk management activities. Communicate resident progress or declines to nurses and supervisors. Promote and ensure residents options and choices for hygiene and care. Ensure admission, discharge, and transfer processes are completed and documented. Complete audits on documentation in the Plan of Care, including Basic Safety Objectives. Assume responsibility for personal and professional growth through staff development programs and committees. Audit therapeutic tubs and ensure they are cleaned and disinfected between uses. Ensure PSWs use equipment and supplies responsibly and complete all checklists. Recognize diversity and support residents rights and values. Demonstrate respect for coworkers, residents, and visitors in accordance with the Code of Conduct. Contribute to person and familycentered care through compassionate communication and validation. Audit units to ensure cleanliness, tidiness, and safety. Timely report all resident and employee incidents or complaints. Maintain privacy and confidentiality at all times. Demonstrate understanding of the Residents Bill of Rights. Perform duties in accordance with Infection Prevention and Control standards, policies, and procedures. Advocate on behalf of residents and ensure their privacy and property are respected. Work in areas that may involve residents with responsive behavior challenges. Physically perform push/pull tasks, work at or above shoulder height, and stand or walk for extended periods with frequent bending and twisting. Follow safe work practices, participate in all required occupational health and safety training, report hazards, and use personal protective equipment as required.
