Responsibilities: Evaluate and assess patients physical conditions and develop individualized treatment plans. Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination. Educate patients and caregivers on exercises, safety precautions, and self-care strategies. Collaborate with an interdisciplinary team to provide holistic, patient-centered care. Maintain accurate and timely documentation of patient progress and treatment plans. Qualifications: Active Physical Therapist license in Oregon, or the ability to obtain. Master or Doctorate degree in Physical Therapy from a CAPT accredited school. Minimum of 1 year of experience as a Physical Therapist. Home health experience is strongly preferred. Fluent in English. Current BLS, handson CPR certification. Reliable transportation with a valid drivers license and auto insurance. If education is from a nonCAPT accredited school, an evaluation by a credentialing agency, such as FCCPT, is required. #J-18808-Ljbffr
