Overview Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) is pleased to partner with various agencies in Dufferin to offer services to the local community at the new Dufferin-based Homeless and Addictions Recovery Treatment (HART) Hub . HART Hubs are community-based care hubs designed to address mental health, addiction and housing needs for people experiencing homelessness. They provide integrated care, offering a wide range of services such as primary care, mental health services, addiction care and employment supports. With targeted community outreach and rapid referral pathways, HART Hubs aim to support individuals with complex needs, helping them recover, achieve housing stability, and inclusion. As part of a multi-disciplinary team, the Intensive Case Manager will provide brief mental health counselling and case management support and deliver time-limited, client-centered interventions. Status: Permanent full-time, 37.5 hours per week. Availability between 8am-9pm Monday-Friday, occasional weekend shifts between 8am-8pm. Note, this posting is for an active vacancy. Salary: $61,201.01 - $72,001.19 annually. (Candidate qualifications will be considered when determining the starting salary) Location: This position is based out of 30 Centre St, Orangeville. Due to the nature of this program, this role is on-site and/or community based. What You Will Do: Collaborate in developing and implementing individualized wellness plans with clients Use standardized assessment tools designed to improve and/or stabilize clients physical and mental health Offer a strength-based approach that recognizes clients unique needs Provision of case management services including but not limited to assistance with activities of daily living, conflict resolution, crisis avoidance and mental health counselling Provide comprehensive, holistic, trauma-informed and person/family centered access to services that address the social determinants of health Partner with clients and their families as members of the health care team Develop and maintain a community resource network Refer to partnering organizations and provide linkages to community supports Work independently and collaboratively within a multidisciplinary team Understand and integrate a philosophy of person and family centered care into daily responsibilities Demonstrate a strong commitment to quality care and service delivery based on the HART Hub Mission and Values Facilitate groups, including the creation of new groups as needed and acting as a back-up facilitator What You Need to Join Our Team: Post-secondary education in Social Work, Health Services, Psychology or equivalent Minimum 2 years experience providing Mental Health case management or equivalent care Demonstrated knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis, and experience with assessment Experience working within the homelessness sector Knowledge of evidence-based practice for mental health services Strong interpersonal skills with proven ability to approach, listen, build trust, and communicate across diverse cultures and intersectionality. Willingness to continually improve understanding around cultural competence. Openness to operate from an anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional framework. Willingness to learn and apply practical understanding of different elements of diversity, such as gender identity/expression, race/cultural background, religion, sexual orientation, disability, immigration and precarious status, family status, etc. Fluency in a second language; ASL is considered an asset.
