Position Type: Temporary Part-Time (up to 3 months) Shift Type: Days-Evenings, some weekends (shifts may be subject to change) Hourly Pay Rate: $48.21 - $64.50 Bi-Weekly Hours: 60 Hours, 7.5 hrs/shift Posting Number: 9270 Union: OPSEU 583 Date Posted: September 19, 2025 Internal Closing Date: September 27, 2025 Job Location: Toronto, Ontario, Canada Job Summary Baycrest Hospital Pharmacy Services support all inpatient units and programs, ensuring the safe and effective use of medications across the organization. The departments key functions include purchasing and inventory management of medications, management of the unit dose drug distribution system, and operation of a centralized intravenous admixture program. The hospital uses an integrated computerized information system (Meditech) to optimize medication management. Pharmacists are integral members of the healthcare team, contributing to clinical activities such as medication reconciliation, interprofessional team rounds, quarterly medication reviews, and mentoring students through preceptorships. The hospital maintains strong affiliation with universities and colleges, offering on-site practical training opportunities for pharmacy students. Responsibilities Participates in relevant rounds and care conferences to ensure safe and appropriate prescribing decisions, patient outcomes are monitored, and adverse drug events are prevented. Identifies, prevents, and resolves drug-related problems by conducting medication reviews and developing/monitoring patient care plans. Communicates care plans to the interprofessional team, patient, family, and/or Substitute Decision Maker. Ensures continuity of care by conducting medication reconciliation when patients are transferred to or discharged from Baycrest. Ensures appropriate and accurate preparation and dispensing of medications to designated hospital programs. Participates in departmental medication management projects and pharmacy clinical programs. Provides education to pharmacy students as per the requirements of the Faculties of Pharmacy (University of Toronto and University of Waterloo). Provides oversight and direction for pharmacy technicians. Ensures best practices are implemented and maintained to enhance medication safety and patient safety. Qualifications Current and valid license to practice as a Pharmacist from the Ontario College of Pharmacists (Part A). Minimum of 2 years of hospital pharmacy experience. Hospital Pharmacy Residency from an accredited institution or Doctor of Pharmacy Degree. Demonstrated expertise in the provision of pharmaceutical care. Self-motivation, excellent communication and organization skills. Computer skills including Microsoft Office (Excel, Word & Outlook). Experience with Meditech preferred. Well-developed critical appraisal, critical thinking, attention to detail, and analytical approach. Demonstrated ability to work effectively as part of an interdisciplinary team.
