Client Care Coordinator (RN/RPN) at Fraser Health Authority Salary: CAD $49.20 - $67.08 per hour. Location: New Westminster, B.C. Job Summary We are hiring a Temp Full Time Client Care Coordinator (RN/RPN) to join our team in New Westminster. Responsibilities Provides leadership to the interprofessional team, coordinates client care activities, and supplies information and consultation to designated care providers and students. Supervises and directs staff: scheduling, coordinating, and adjusting work activities; establishing priorities; providing feedback on performance. Evaluates the provision of care to clients and families in accordance with the organizations vision and values. Participates in safety programs, ensuring the safety of clients and staff. Coordinates quality improvement and risk management activities, including evaluating changes in clinical practice and tracking critical incidents. Provides staff development support, including orientation and ongoing education. Assists in reviewing client records for legibility, completeness, and accuracy. Consults and collaborates with interprofessional team members on client/family issues. Participates in committees and task groups to facilitate program goals. Promotes researchbased practice and supports the development of ongoing research. Assists the Manager with planning, developing, and evaluating client care, including input on clinical practice and systems. Provides clinical leadership and consultation to clinical staff and students. Assists with administrative activities and optimal use of client care resources. Provides consultation and/or screening of clients in special circumstances. Sets measurable personal performance goals and objectives. Assists staff with client care activities as required. Oversees building maintenance requests and implements health and safety plans. Performs other related duties as assigned. Qualifications Education, Training and Experience Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM); three (3) years of recent related clinical experience, including one (1) year in a supervisory role. Completion of substance use certificate program required. For licensed facilities: St. Johns Ambulance Safety Orientated First Aid Certificate and current CPR level C or equivalent. For Mobile Integrated Crisis Response (MICR) only: Enhanced Security Police Clearance required. Skills And Abilities Clinical proficiency/expertise in the designated clinical area. Knowledge of community resources and healthrelated support services. Ability to supervise and provide leadership and work direction. Ability to coordinate client assignments. Ability to monitor staff workload and patient factors. Ability to establish priorities for client care and overall daily operations. Ability to monitor the quality of client care and ensure maintenance of standards. Ability to assess and address performance issues and provide input to formal reviews. Ability to model, coach, and mentor best practice. Ability to identify learning priorities and support implementation of education. Ability to provide input into budgets and equipment. Ability to communicate effectively both verbally and in writing. Ability to work independently, organized and selfdirected. Ability to problemsolve and manage conflict.
