About the Role Reporting to the Program Manager, Healthy Living, this position is responsible for working with a high degree of autonomy to prevent illness, promote and protect the health of individuals, families and the community throughout the life span, within the parameters outlined by the Ministry of Health, Ministry of LongTerm Care, Ministry of Children, Community and Social Services, Health Protection and Promotion Act (HPPA), Ontario Public Health Standards (OPHS) and Protocols and other relevant legislation; providing enhanced supports by identifying specific priority populations impacted most negatively by the determinants of health, increasing capacity to service and support priority populations and addressing health inequities through divisional programs and focus areas of service delivery. What Youll Be Doing Provides health promotion and protection activities, including assessment, early detection and intervention, clinical services, educational strategies, disease surveillance and investigation, consultation and counselling, and harm reduction strategies, in order to enhance the wellbeing and prevent the occurrence of illness of individuals, families and communities or to return them to optimal health. Provides services and programs in a variety of settings including homes, workplaces, schools, childcare settings, organizations, institutional settings and clinics. Provides leadership in promoting community activation strategies in order to increase the communitys knowledge of and ability to influence determinants of health. Participates in the development of public policy and social marketing strategies to advocate for and promote healthier communities. Collaborates with community partners in the development and delivery of health promotion and protection activities. Plans, implements and evaluates health promotion programs, services and strategies based on needs assessments and in compliance with legislative requirements. Reviews relevant literature and interprets data (e.g., demographic profiles, epidemiological trends, service gaps, consumer attitudes) in order to recommend implementation, monitoring and evaluation strategies. Considers York Region strategic plans, programs, research and services in the development and delivery of health promotion programs. Participates in the development and implementation of strategies, program logic models, operational plans, work plans, evaluation and quality assurance strategies to support effective program delivery. Knowledge of principles of needs assessment, program evaluation and continuous quality improvement. Performs other duties, as assigned, in accordance with Department, Branch and Divisional objectives. What Were Looking For Successful completion of a Baccalaureate University Degree in nursing, including public health. Current Certificate of Registration as a Registered Nurse from the College of Nurses of Ontario. Certified in CardioPulmonary Resuscitation Basic Life Support (CPR BLS) with certification having been completed within the last 12 months. Satisfactory Police Criminal Background Check. Valid Ontario Class G drivers licence and reliable vehicle for use on corporate business. Knowledge and demonstrated ability in corporate core competencies including communication, innovation, teamwork and collaboration, and personal ownership. Knowledge of population health, social determinants of health, resiliency and the ability to utilize current theories and best practices to develop effective health promotion strategies in the specific program area. Ability to consult with individuals and groups to make recommendations related to policy development and implementation.
