In addition, the position of Health Information Management Professional is to abstract and code patient information based on ICD-10/CA/CCI. The Health Information Management Professional is responsible for the interpretation of source documents located in the patient medical record and applies code assignment by selecting and assigning numeric codes from the classification systems to represent diagnostic and intervention data. Accurate code assignment involves interpretation and application of coding instructions, guidelines, conventions, standards, policies, rules and ethics.Position RequirementsPrepare and process personal health informationProcess correspondence and release personal health informationRespond to the information requirements of Coroner's investigations, situations of child abuse, transfers, and other urgent situationsBe familiar with statutory and legislative requirements of personal health informationCode and abstract patient information in accordance with CIHI guidelines, hospital policies and procedures, and established hospital guidelinesInput, process, update, correct, verify, monitor requests for follow-upRead and interpret health record documentation to identify all diagnoses and proceduresCommittee and Care Team involvementMandatory QualificationsMust have a CHIM certificationHighly motivated and customer orientedAbility to co-ordinate multiple initiatives effectively, including the initiation and positive paradigm of changeKnowledge of computerized: Admissions, MRI, Abstracting Grouper and Report Writer SystemsStrong organizational skillsExcellent oral, interpersonal, and written communication skillsIntermediate computer skills, utilizing Microsoft software such as Word, Excel, PowerPoint, OutlookCanadian College of Health Information ManagementCurrent registration with the Canadian Health Information Management Association (mandatory)Ontario Health Information Management Association (optional)Primary ResponsibilitiesSafety (patient, worker & workplace) is a BCHS Corporate Priority.
