About the role As part of our MOBILE HEALTH BUS, the Nurse Practitioner (NP) will be an integral part of a team that brings primary care, brief case management and harm reduction services to homeless and precariously housed individuals with complex mental health and substance use issues in the Downtown East community. Scope of the mandate The NP will provide a full scope of primary care and liaise with other Health Bus team members and external health care providers and community partners to ensure the delivery of comprehensive health care to clients on the Health Bus and complete follow up at Sherbourne Health and in the community as required. The NP works within a health promotion framework, collaborates within a multidisciplinary team, and meets the standards of practice prescribed by the College of Nurses of Ontario. Responsibilities and Tasks CLIENT SERVICE: Provide episodic primary care considering environmental, social, psychological and medical risk factors, explained clearly to clients. Discuss health concerns with clients and determine an appropriate plan of care. Provide ongoing follow-up and evaluation of the plan of care. Offer supportive counseling for mental health/emotional concerns and refer to internal or external resources as needed. Collaborate with Health Bus team members and refer clients to other services to ensure comprehensive, competent care. Assess clients for routine health and illness care and prescribe treatment as appropriate. Coordinate sexual health visits, cervical screenings and other clinics as needed. Consult or refer for further assessment or treatment when appropriate or beyond NP scope. Prescribe medications and order diagnostic tests within NP scope of practice. Advocate for clients and use health resources effectively. Deliver care within the NP scope of practice as outlined by CNO and Bill 179. Provide follow-up after referrals to other primary care services. Ground primary healthcare in the five principles of Primary Healthcare as outlined by WHO. Work from a harm reduction approach, including Naloxone and harm reduction supplies as needed. Identify barriers to health and facilitate access to resources. Ensure a healthy and safe workplace in compliance with policies and laws. Health Promotion and Education Participate in development, planning and evaluation of the Health Bus program. Counsel and educate clients on health-related topics and promote mental and physical health. Facilitate health promotion programs and collaborate with community partners to improve access to primary health care. Engage in networks and coalitions focused on homeless/under-housed populations. Mentor NP and RN students and staff; engage in evidence-based practice and quality improvement. Collaborate to address population trends, research questions, and knowledge translation. Advocate for clients to improve access to care and resources; participate in systemic advocacy for health equity. Education Pursue ongoing professional education; complete minimum hours of continuing education per year as required by CNO. Participate in training of other healthcare professionals. Administrative Duties Identify where protocols and procedures can improve services or staff functioning. Participate in meetings with Health Bus and partner organizations. Maintain accurate client records in the EMR and document encounters for data collection. Assist team with administrative and clinical duties as required. Other duties as required. To thrive in this role, youll need education/experience: Current registration as a Registered Nurse in the Extended Class with the CNO (an NP with Extended Class Certificate). Current Basic Life Support (BLS) certification. Evidence of completion of CNO-approved controlled substances education for NPs. Minimum 2 years of clinical NP experience and at least 3 years as an RN. 2-3 years of primary care/community setting experience (asset). Experience in gender-affirming care and working with marginalized populations using trauma-informed, anti-oppressive and harm reduction practices. Understanding of PHIPA, RHPA, CNO standards; ability to comply with relevant legislation. Knowledge of health care systems and social determinants of health; ability to work across teams. Experience helping clients access primary care/community services in Downtown Toronto; experience with mental health and substance use issues. Harm reduction experience, including OAT and safer opioid supply (asset). Skills and abilities Ability to work independently and in a multidisciplinary team. Strong interpersonal, crisis intervention, leadership, organizational, problem-solving, decision-making, and communication skills. Experience with EMR systems and MS Office; teamwork and anti-oppression mindset. PHIPA, RHPA, CNO standards knowledge; professional practice alignment. Ability to speak additional languages (asset) and experience providing culturally safe care to diverse populations (asset). Valid G-Class Drivers License and clean driving record to operate the Health Bus (required).
