Your Opportunity This is an exciting leadership opportunity within the Pediatric Community Rehabilitation (PCR) program. The successful candidate will join a passionate team committed to delivering high-quality, family-centered care to children and youth across multiple sites. Services are provided by a multidisciplinary team supporting children with diverse needs. Reporting to the Program Manager, the Care Manager provides day-to-day leadership, clinical coordination, and operational support to interdisciplinary staff, including Occupational Therapists, Physiotherapists, Speech-Language Pathologists, Therapy Assistants, Social Workers, Resource Personnel, and Dietitians. This role ensures service delivery aligns with program priorities, supports staff in daily operations, and fosters a collaborative, client-centered approach. The Care Manager works closely with clinical leaders and other discipline leads to implement a coordinated service model and integrated care across the continuum. Responsibilities include onboarding, caseload coordination, rollout of provincial updates, and communicating service delivery challenges to leadership. Description The Care Manager fulfills a leadership role in Allied Health and reports directly to the Pediatric Community Rehabilitation Program Manager. This position is responsible and accountable for providing the direction, team leadership and organization for the Occupational Therapists, Physiotherapists and Speech Language Pathologists who provide services to children and youth. The position requires effective communication with strong organizational and interpersonal skills, proven ability to collaborate and foster partnerships, family/client centered focus, commitment to providing quality services, demonstrated ability and comfort with decision making responsibilities, coaching and teaching, the ability to inspire and build confident in others, ability to manage change, and manage human and physical resources. Masters degree in Health Administration or equivalencies of education and experience will be considered. Active membership in good standing within profession. Previous leadership experience in an organization of significant size, complexity and diversity. A clear pattern of professional and personal development.
