INTRODUCTION:As part of the Primary Care Team, the Social Worker provides essential wholistic support to patients through case management,health promotion and patient advocacy to address the social, emotional, financial and spiritual factors that impact their health.Theyact as a vital member of the interdisciplinary team, bridging the gap between a patients medical needs and their broader lifecircumstances.REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:Assess patient needsProvide referrals to community resources and servicesProvide emotional support, understanding and encouragement related to treatment decisions and diagnosisProvide supportive counselling as requiredDevelop and lead health promotion activitiesLiaise with other departments within the organization, external community agencies and service providersDevelop and deliver education to individuals, families and communitiesPerform community health needs assessments and participate in community health planningAdvocate on behalf of patients, families, groups and communities as neededProvide case management support for patients accessing primary careSupport complex discharge or transition planningFollow up on referrals, screening and adherence to follow-up care and health care provider recommendationsCoordinate social services and complex referrals to facilitate information exchangeAttend and participate in client conferences and complex case resolution with the interdisciplinary teamFacilitate meetings with care team members, clients and families when neededDevelop patient resources as requiredProvide all care from a patient centered, trauma informed approachStrong interprofessional communicationSTATEMENT OF QUALIFICATIONS:Bachelor or Master degree in Social WorkMember in good standing with the Ontario College of Social Workers and Social Service WorkersExperience working with the Indigenous communityExperience working in Primary Care or part of an Interdisciplinary TeamExcellent interpersonal skillsExcellent communication skills, written, oral and presentationKnowledge of the Indigenous community, their history and impacts on healthKnowledge of and proficiency completing required documentation using electronic medical records and softwareWorking knowledge of the Mental Health Act, Health Care Consent Act, Substitute Decisions Act and communityresourcesINTERESTED APPLICANTS:Please submit your resume and cover letter to: :weh / Miigwetch to all applicants.
