Position summary: As a member of the Management Team the Patient Care Manager is accountable for leading initiatives and implementing change on the patient care floors with the goal of accountability, respect, excellent patient care, clinical best practices and innovation. The Patient Care Manager is responsible for the overall management of the patient care people, financial, quality, risk and clinical resources that are required of the assigned patient care units to provide quality patient care. Primary Responsibilities: Accountable/responsible for the oversight and day to day operation of the patient care unit including human resource and financial responsibilities of the patient care floor. Ensures organizational initiatives are implemented and completed in accordance with the strategic direction of Runnymede Healthcare Centre (Runnymede), applicable legislation, corporate policies and collective agreements and may be required to lead initiatives as assigned. Work collaboratively with other leaders and take joint accountability for achieving performance targets, improving quality and safety and advancing patient care outcomes under the direction of Director of patient care. Monitor, identifies and responds timely to quality, safety and risk issues; escalates were necessary. Delegates the implementation of initiatives for after-hours to the Site Operation and Practice Leader (SOPL) and/or Advance Practice Nurse (APN) to ensure consistent messaging to all clinical staff. Ensures processes are in place and that all team members fulfill the requirements and accountability of their positions. Takes action to ensure patient and staff safety by completing safety walkabouts and investigating incidents and making recommendations for improvement. Identifies issues and challenges, monitoring trends requiring targeted intervention and change in practice and processes. Coach and mentor, facilitates and supports the creation and maintenance of an environment that fosters trust, accountability, productivity and cooperation among interdisciplinary team. Conducts monthly staff meetings. Attends mandatory in-services, participates in committees, task forces and RHC events as requested Maintains a visible presence on the floor. Leads and oversees unit based improvement initiatives to maintain or improve quality patient care Uses conflict resolution methods to resolve team and patient issues. Oversees and ensures staff compliance with documentation and other requirements as per Runnymede and the College standards of practice via audits and other processes. Analyzes data/information of quality, utilization and outcomes indicators and plans initiatives to improve the indicators. Oversees clinical workload on the patient care floor to ensure optimal productivity, safety and service levels. Prepares and submits accurate and timely written reports for committees and assigned projects. Attends Patient Family Council Meetings representing Patient Care Management if necessary Responsible for recruitment, disciplines, termination, performance reviews and manage the staff according to Runnymede policies and collective agreements. Asses, implements, and monitors attendance management program. Accountable for unit staffing and skills mix and delegates daily staffing and scheduling. Fosters effective labour relations through understanding and implementation of collective agreements Accountable for monitoring, controlling the budgets and resource utilization at the unit level and monitors and reports on variances in budget and patient care statistics. Completes policy/procedure review and development as required. Assumes a leadership role in emergencies, hospital-wide issues, crisis situations (e.g. Code Blue, Code Red etc.). Monitors and ensures adequate clinical supplies and inventory and makes recommendations for efficiencies. Accountable for own personal professional development through participation in professional associations, relevant courses, workshops, research and reflective practice. Performs on-call administrative duties as assigned The successful candidate will possess the following qualifications/experience: Masters in Nursing, Health Administration or Health Related Field Preferred Must possess valid and current license from College of Nurses of Ontario (in good standing) 5 years'' clinical experience in an Acute Care or Rehabilitation/Complex Continuing Care Hospital required 3 years recent managerial or leadership experience preferred Active involvement in own continuing education and professional association Proficient with Windows Operating System, Microsoft Word, Microsoft Excel, Microsoft Power Point Demonstrated ability to effectively communicate with all levels of staff, physicians, patients, and families While we thank all applicants, only those under consideration for the position will be contacted.
