In this role, youll manage internal and external coordination - handling referrals, follow-ups and care transitions while also building trusted relationships with each client.This role is ideal for someone with a clinical or health navigation background who brings a detail-oriented mindset and a passion for advocacy and continuity of care.Key ResponsibilitiesAct as a liaison between clients, specialists, hospitals, labs, and community providersSchedule, track, and follow up on specialist referrals, diagnostics, and proceduresSupport individualized care plans and participate in interdisciplinary case reviewsConduct follow-up calls and occasional external visits as neededProvide client education on diagnoses, medications, and care plansEnsure EMRs are updated with accurate appointment, referral, and outcome detailsSupport insurance-related processes such as pre-authorizations and verificationServe as the main point of contact for care-related inquiries and coordinationQualifications & ExperienceRegistered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of OntarioBachelor of Science in Nursing (BSN), or equivalent health navigation backgroundOther relevant backgrounds may include: patient navigator, medical social worker, health coach, or private care conciergeMinimum 3 years of experience in care coordination, case management, community health or outpatient supportStrong understanding of Ontarios healthcare system, pathways, and provider networksExceptional communication, documentation, and organizational skillsProficiency in EMR systems and digital clinical workflowsAdditional training in chronic disease management is an assetCore CompetenciesClient advocacy and relationship-buildingMultidisciplinary care coordination and follow-upHealth coaching and client educationClinical documentation and workflow managementInsurance and referral navigationCultural sensitivity and PHIPA/HIPAA complianceWhy Join Stem Health?Competitive compensation, a collaborative, innovative, and supportive team cultureAccess to on-site diagnostics, health coaching, and wellness servicesA modern, high-end clinic space that fosters calm, comfort, and confidentialityOpportunity to help shape a new model of premium primary care in CanadaIdeal CandidateYou bring warmth, clinical insight, and a commitment to exceptional follow-through.
