The team includes pediatric urology, adult urology, and nurse practitioners with primary care training, fostering a collaborative and holistic approach to patient care. The position offers flexibility in FTE and care delivery methods, including virtual and in-person options.BackgroundThe Ledcor Centre for Adolescent and Transitional Urology is an innovative initiative aimed at enhancing the transition from pediatric to adult healthcare for patients aged 15-25, primarily managed by pediatric urology and related clinics. The nurse practitioner will serve as the primary contact for these patients, providing assessment, management, and coordination of their healthcare needs.Role of the Transition Clinic Nurse PractitionerThe NP will co-manage patients, addressing acute concerns, medication management, mental health issues, and facilitating navigation within the adult healthcare system.
