In addition, Yee Hong offers four campuses of managed care, a hospice residence, and a wide range of community-based senior and caregiver support services. Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area. Program Coordinator, Home Support Services - Temporary Full Time for about 12 months, with possibility of extension Core Function: The incumbent is responsible to the Program Manager/Director of Home Support Services, for the overall day to day coordination of Support Services to the community. Qualifications: Bachelor''s degree in social services, health care, activation, or equivalent Minimum 2 years experience in social services or health care, supervisory experience as an asset Knowledge and experience in working with seniors Current CPR Certificate required 5. Satisfactory Police Reference Check (with vulnerable sector screen) result Ability to communicate in English (Ability to communicate in Chinese as an asset) Responsibilities: Coordinates the delivery of appropriate home support services to enable clients to age in place Coordinates meal and transportation services for clients Provides timely and professional client intervention to clients and family Monitors overall health status and keeps up-to-date health records of clients Coaches, develops and evaluates staff to ensure they meet their required performance standard. Accurately schedules staff resources in accordance with clients'' needs as reflected in the care plan Ensures implementation of risk management policy and safe environment for clients and staff In consultation with the Manager, develops and updates policies, procedures for the program Coordinates the optimal deployment of volunteers for the program and provides effective guidance, training and support to the volunteers Maintains up to date and accurate files, records and statistics on the program Ensures efficient and responsible use of resources and maintains accurate record of fees and expenditures Provides emergency response to emergency calls from staff and clients Effectively promotes the home support services to clients of the building/neighbourhood Keeps up-to-date with developments and best practices in the field Other duties as assigned Physical Demands/Working Conditions: This position is temporary Full Time for about 12 months, with possibility of extension Light Working Normal Occasional travel Use of own vehicle Please apply before: Starting Rate: $33.28 Salary Range: $ $39.15 INTERNAL APPLICANTS: PLEASE APPLY THROUGH THE JOBS HUB Our people are our greatest strength and the core of our success.
