Duties Assess the social, emotional, and environmental needs of patients, especially children in pediatric and hospital settings Develop individualized care plans in coordination with medical teams and families Provide counseling and emotional support to patients and their families during difficult times Assist with case management, including coordinating services such as special education or community resources Collect, document, and analyze data related to patient progress and service effectiveness Advocate for patient rights and connect families with appropriate resources and support systems Collaborate with multidisciplinary teams to ensure holistic patient care Support research initiatives aimed at improving pediatric healthcare practices Requirements Experience working with children in healthcare or social service settings is highly preferred Prior hospital experience is advantageous Knowledge of medical terminology relevant to pediatric care and patient treatment processes Strong background in case management, patient care, and data collection techniques Familiarity with research methodologies related to healthcare or social services Understanding of special education needs for children requiring additional support Excellent communication skills, empathy, and ability to work effectively within multidisciplinary teams Ability to handle sensitive situations with professionalism and compassion This role is vital for ensuring that patients receive comprehensive support tailored to their unique needs.
