Job Description Baycrest Long Term Care has an opportunity for a Personal Support Worker (PSW) Apotex 5 PERMANENT PART TIME, DAY Posting Number: 9489 Shift Type: Day, alternate weekends ( subject to change) Hourly Pay Rate: $26.37- $27.05 Bi-Weekly Hours: 30 Hours, 8 hrs/shift Union: SEIU Service Entity: Baycrest Hospital Date Posted: December 16, 2025 Internal Closing Date: December 29, 2025 Apotex 5 is a Cognitive Support Unit. Residents residing on this floor are living with a wide range of cognitive and physical changes to their abilities from mild to severe cognitive impairment and from ambulating independently, to being wheel-chair bound. Responsibilities include but are not limited to: Supports, in consultation with the RN/ RPN, the physical and personal care needs of the client including, but not limited to: hygiene, grooming and dressing, toileting, recording nutrition and hydration intake, provision of preventative skin care Supports the social, emotional, cognitive and cultural well-being of the client under the guidance of the direct care team Uses professional judgment, monitors and evaluates client care needs and care goals regularly according to LTC standards, policies, procedures and legislation Initiates and maintains a relationship with the client, their family and other care providers Qualifications include but are not limited to: Current P.S.W. certificate from a recognized community college or educational institution is required Recent placement and/or working experience with the physically and/or cognitively impaired elderly in a health care setting is required Experience with mental health assessment and intervention and working with responsive behaviors due to various types of dementia, delirium, and other mental health issues is preferred Basic computer skills Meditech/PointClickCare, Microsoft Word and Excel is an asset Ability to work independently and as a part of the interdisciplinary team, and effectively manage challenging situations Excellent interpersonal, communication and problem-solving skills Additional Benefits: % Vacation Entitlement Opportunity to enroll in the extended health and dental benefit plan Opportunity to enroll in the Healthcare of Ontario Pension Plan (HOOPP) Access to 24/7 Employee Assistance Program INTERNAL APPLICANTS: Current Baycrest employees must apply online through the Baycrest intranet.
