Job Summary We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will provide high-quality patient care. Duties Deliver comprehensive patient care in accordance with medical plans and protocols Administer medications and treatments accurately following established procedures Monitor patient vital signs and assess health status regularly Assist with specialized care such as dementia management and pain control. Perform laboratory specimen collection as needed. Document patient information accurately using electronic health record systems. Collaborate with physicians, therapists, and other healthcare professionals to develop and implement care plans Educate patients and their families about health conditions, medications, and post-discharge care Support case management activities to coordinate services for complex or long-term patients Maintain a safe and clean environment adhering to infection control standards Requirements Valid registration as a Practical Nurse with the appropriate licensing body Proven experience in nursing home, hospital, or home care settings preferred Knowledge of medical terminology, medication administration, and patient care procedures Experience with electronic health records is an advantage * Strong communication skills and ability to work effectively within a multidisciplinary team Ability to handle sensitive situations with compassion and professionalism Experience with pain management, case management, or assisted living environments is beneficial Flexibility to adapt to various patient needs.
