The Opportunity We are currently seeking a compassionate, organized, and community-driven Community Support Worker (CSW) to support patients in Osoyoos through a partnership with Interior Health and local medical clinics. This is a full-time, 33.75 hours/week position that offers a dynamic work environment and the opportunity to positively impact lives every day. What You''ll Do Working as part of a team-based care model, the CSW will serve patients from Osoyoos Medical Clinic and Desert Doctors Clinic. Your responsibilities will include: Providing outreach-based support to patients referred by physicians and other healthcare providers Assisting clients with completing government forms (e.g., PWD, CPP, Income Assistance, MSP, etc.) Offering life skills support and guidance to patients and their families Attending medical appointments with patients to ensure continuity of care Facilitating referrals and connections to community services and supports Advocating for clients and helping them navigate complex systems with dignity and respect What We''re Looking For Previous experience working with vulnerable populations and navigating community/social services Strong interpersonal and communication skills Reliable, self-motivated, and comfortable working independently in the community Compassionate, non-judgmental, and organized Valid driver''s license and reliable transportation (mileage reimbursed) Why Join Desert Sun?
