Job Summary We are seeking compassionate and dedicated Support Workers to provide high-quality patient care across various healthcare settings. The Support Worker will play a vital role in delivering personalized care, supporting treatment plans, and maintaining a safe environment for patients of all ages. Responsibilities Assist patients with daily living activities such as bathing, dressing, mobility, and feeding. Support medical procedures including medication administration and basic patient assessments. Document patient information accurately using electronic health records systems. Monitor patient conditions and report changes to healthcare professionals promptly. Provide specialized care for patients with dementia, hospice needs, or requiring pain management. Collaborate with nursing staff and case managers to develop and implement individualized care plans. Maintain a clean and safe environment adhering to infection control protocols. Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care delivery. Qualifications Prior experience in nursing homes, hospitals and home care settings is preferred. Knowledge of medical terminology, medication administration, and patient care procedures. Experience with electronic health records system. Certifications in PSW , First aid and CPR, RPN diploma Strong understanding of acute care, dementia care, pediatrics, senior care, hospice care, and pain management is desirable. Excellent communication skills and compassionate approach to patient support. Ability to work effectively within multidisciplinary teams including case management professionals. Experience working with children or in assisted living environments is beneficial.
