Location: Grace Anglican Church, Waterdown 157 Mill Street North, Waterdown, Ontario Full Job description: Part time 7.5 hours a week Wednesdays from 9 am-4:30 pm Come and work with Awesome, Amazing, Able young adults Our Adult Day Program Team Member assists with the planning, facilitation, implementation, evaluation and documentation of social and recreational programs for clients attending the Adult Program; providing and assisting clients with activities of daily living (ADLs) to maximize their health, well-being and independence. With a ratio of 1: 3 you will have a chance to really get to know the Graceful Abilities participants who you are caring for and develop appropriate participant focused activities for them to socialize and develop independence. We currently enjoy cooking, using computers, fitness class, Life Skills, scrapbooking, and exploring Waterdown and Hamilton by bus and train. The group includes wheelchair users, some participants with mobility issues, verbal and non-verbal communicators. The team at Graceful Abilities and Grace Anglican Church will work together to support you in providing exceptional care for each participant. Certification in the following will be provided unless you already have it: First Aid Safe Food Handling Our site (Grace Anglican Church, Waterdown) is fully accessible and includes Bobby''s Place (a bright new extension with an accessible washroom and kitchenette), Noble Hall, Duby Room, a commercial kitchen, and a cemetery with community gardens. There is plenty of space for activities and for calming, when needed. Graceful Abilities announces the opportunity for 1 Part Time Adult Day Program Supervisor for 7.5 hours on Wednesdays. The employee will work at Grace Anglican Church, Waterdown and off site on occasion. The position is from 9:00am 4:30 pm on Wednesdays. Primary functions: Support the delivery of the activities associated with recreation programs that meet the client needs in accordance with program policies, procedures and practices. Assist with the planning of the programs by providing inputs in development of all recreational, social and life skills programs including, but not limited to: providing input into monthly calendar and completing program plans for all group programs. Assist clients with activities of daily living including, but not limited to: serving snacks, medication reminders, washroom, personal care and mobility assistance. Complete all essential documentation thoroughly and within time requirements, including but not limited to: progress notes, incident reporting, attendance records, program plan and program evaluation. Report any concerns brought forward by the client/caregiver or related to safety to the Program Supervisor in a timely manner for follow-up. Assist with maintaining all public health standards and inspection documents. Maintain effective communication with the team (staff, volunteers, clients and caregivers and community resources.) Participate in education opportunities as appropriate Other Duties Assists with registration and other forms. Compiles program reports on activities and milestones. Maintains accurate records and prepares written reports as requested. Undertakes cleaning when required. Performs other related duties as required. TERMS OF EMPLOYMENT This position includes work on weekdays; some off-site work may be required. This position will begin with a three-month probationary period. A satisfactory Criminal Reference Check with vulnerable sector screening is required. This position reports to the Rector and Wardens and is supervised by the Rector.
